For referrals to our home visiting service , please complete one of our referral forms.
To be eligible for our free support families need to have at least one child under school age and be living in the Wokingham Borough.
Professionals
If you are a health professional or social worker and want to refer a family for support please use this referral form.
Parents
If you are a mum or dad and want to self-refer please use this referral form.
Once we receive the referral form we will be in touch to find out more about the family’s needs so that we can find the best way to support them. We have a huge demand for our services often have a list of families waiting for help and the services we offer may not be the right solution for all families. For these reasons and due to limited resources, we cannot always help every family. Where appropriate we will always sign post families to other local services who may be better placed to help.
